Ascension Personalized Care was the ACA individual and family plan built by Ascension, one of the largest Catholic nonprofit health systems in the country, and sold as an EPO that routed members to Ascension hospitals and physicians. Coverage was underwritten by US Health and Life Insurance Company, an Ascension company writing employer benefits since 1996, and was available in Indiana, Kansas, Tennessee, and Texas. The plan earned URAC Marketplace Health Plan Accreditation and was never assigned an AM Best financial strength rating. Ascension withdrew the product from the marketplace, on and off exchange, effective January 1, 2025, and the final day of member coverage was December 31, 2024. If you held an APC policy, you will need a replacement individual plan.
